Registered Nurse Case Manager Home Care Mercy Health

Job Summary
The Case Manager is a registered nurse who applies nursing knowledge and clinical skills using the nursing process to ensure quality and safe delivery of home care services. The Case Manager possesses strong clinical skills and a knowledge base in a specialized area, and utilizes leadership skills in coordinating and/or providing home care services for a group of patients within that specialty area. These services include education of patients/families and staff, collaboration with the physician in the plan of care, total coordination of patient care and the quality monitoring of appropriateness of patient care. The group of patients can include home care patients of all ages. The Case Manager facilitates achievement of outcomes.
Qualifications - Minimum
Education:
The Case Manager is a registered nurse currently licensed to practice nursing in the State of Ohio. AD, BSN or diploma degree from an accredited program in nursing is required. The Case Manager must have a minimum of 2 years recent acute care or home care experience.

Hours
Hours per pay period: 64
Hours: 8:00am to 4:30pm
Every third weekend with rotation of On-call and Flex Schedule (2:00pm to 10:30pm)

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
